We encourage you to complete the New Guest Paperwork at your convenience to bring with you to your scheduled appointment time. If you choose to complete the paperwork at our office, we ask that you arrive 15 minutes before your scheduled appointment to make sure we have plenty of time to give you the exceptional care we are known for and you expect from us.
If you would like to have your records transferred from your previous office to us, we have provided a records release authorization that gives your previous provider permission to share your information and records with us.
As a convenience to you, we work with a third party to correspond via text message and email. You have the option to opt out of this service at any time.
Should the need arise to reschedule or possibly even cancel a scheduled appointment, we respectfully require 48 business hours notice. Failure to show for an appointment or adequately provide us with appropriate notice will result in a RECOVERY FEE of $75 per scheduled hour. We understand that sometimes there are unforeseen circumstances that may cause a guest to late cancel or miss an appointment. Recovery fees are applied on a case by case basis following careful review of a guest’s account and/or appointment history.
